Wish we could back up our iDevices to Time Machine. This would definitely solve this issue.
Whenever I've backed up my iphone or ipad, and restored, I was under the impression I could choose any restore point, from a dozen choices let's say. But the choices you get at the restore point can be anything from "this afternoon iPad backup" to "nothing" to "there's one here you did two weeks ago" :/ And I'm backing up every day.
So yeah, a little bit confusing to say the least. Hopefully their "major overhaul of iTunes" will help. Ahem...

This article no longer applies. Control-click on item in Devices now just gives the option to backup only. (No custom backup options)
I tried to backup a new iPad 4th gen from a fresh iPhone 4S backup (both on 6.0.1, both 64GB) and I do not get folders - years of Apps strewn across multiple screens - not even in alphabetical order. Apple Support suggested I sort them on the computer - hours of tedious work to get them to match the iPhone layout.
I also tried a complete restore too followed by a sync... hours of wasted time.

Now I know I shouldn't have updated the mini that has all our iDevice backups to iTunes 11. It crossed my mind it might affect it, but you're optimistic things will be better, not worse; so you forge ahead to snag the improvements.

Such optimism is justified - except when it's not. If you back up your system regularly, such disasters will have minimal impact. If you don't back up, it's only a matter of time before you regret your negligence. And who's to blame for that? At the least you should use Time Machine with an external drive. An up-to-date system clone is even easier to restore from. Tired though the aphorism may be, it's true just the same: A stitch in time saves nine. I guess no one teaches such homely wisdom any more.

This article absolutely still DOES apply.
I can't believe that a couple of years have passed, iOS has gone through multiple versions, iTunes has been updated many times, and this idiotic app restore situation still applies!
The fact is, using the latest iTunes revision (on Windows at any rate), displaying the sidebar and then right-clicking the Device name when connected still presents the referenced Restore from Backup option.
And it DOES put all the idiotically scattered apps back into their original folders!
