As I live in Denmark and have a different MacBook (an 2007 MB Pro) I do not have the same setup as you. But no later than yesterday I had trouble getting Bluetooth Hotspot to work even though it worked previously. They connected to each other but I couldn't get on the net. The solution was to move the iPhone (or MacBook) and everything worked fine again. Apparently one of them (or both?) have weak spots in the antenna's coverage.

I haven't really had any issues since iOS 4.0. I currently have 2 different aftermarket Bluetooth car stereos: JVC KW-AVX740 double DIN DVD, and one made by Dual with flip down face and built-in iPhone dock. Both are labeled as "Made for iPhone," though.

The most trouble I had was with 2 different stereos made by Power Acoustik. They didn't like connecting to iPhones.

I have never had success getting my iPhone to work with any rental car. Not talking about Bluetooth here, which the cars don't have anyway, but USB. Despite having a USB port, and clearly labelled as working with iPods, audio systems are unable to deal with iPhones or iPod touches. Tried several different models and makes of cars (compacts and midsize) — all had the same result.

My wife and I are having the same problem with a 2012 Prius and our iPhone 4s's.
Bluetooth worked great when we got the car last September, but now the phones won't connect.
I've tried some of the suggetions on the Apple forums, including resetting network settings on the iPhone, but nothing works.
My call to Apple support today was useless. The tech "hadn't heard of this problem", but declined my offer to send him a copy of this article. He kept insisting I should talk to Toyota to see if there is an update to the car's software. This reminds me of my old experiences with Microsoft support.
Apple needs to post a white paper on this issue and explain what's going on.

Were you able to fix this? We are experiencing the same problem.
