Do a reset of the iOS device (hold down Power & Home buttons, ignore the red swipe and let go when the apple logo appears -- apologies for teaching grandma how to suck eggs).
Turn notifications to ON only on the iOS device.

I did it when I first installed iOS 6 on my 4S. You have to search for the IPSW file online. There should be a handful of sites to download it from.
Then, connect the iPhone to iTunes.
Hold down the Option key then click on Restore.
You can now select the iOS 5 IPSW file.
Click Restore... Done.
WARNING: Apple stopped signing iOS 5 about a week after iOS 6 came. This is probably a no-go at this point. You can give it a try, but it'll probably fail.

I'd recommend you both contact your service providers and complain about the issue and ask them to fix it. I've had this issue in the past, and it boiled down to one of the service providers having some issue.
And if you're on a GSM-based network you can easily confirm that it's not your phone, just put your SIM card into another phone and try sending an SMS to the guy.
