Apple Mac Pro Kernel Panic Problem Report

I started having kernel panics whereas the Mac displays a small black semitransparent window with text in several languages that you must restart your Mac. The Mac started doing this at random points after work times of an hour or more, several times a day. I did a Google search on the first few errors and one forum post suggested it might be an overheating issue. I installed smcFanControl and it said the temperature was 135 F so I doubled the speed of all the fans from their default settings, which is no where near their max. The temperature dropped down to the 91 to 108 range after that and the kernel panics are less frequent; however, they still are occurring once, sometimes twice a day. Any idea what it might be based upon the report below:

It has been my experience that most kernel panics are hardware-related, and most hardware-related kernel panics - especially the ones that happen routinely a significant but predictable amount of time post-boot - are RAM-related. Buried in that log was this: "Machine-specific error: Read ECC." I'd focus my attention on the memory. Especially if you've added or reconfigured it recently.

The first time I attempted to run a Apple Hardware test the Mac appeared to make no further progress shortly after starting the second pass. I retried and during the 1st pass it came up with the following error code:

4SNS/1/40000001:Ip6M-0.953

Google turns up similar, but nothing with the same exact ending number. Searching apple support returns nothing for this specific error.

Symptoms

If you test a Mac Pro (Mid 2010 and later) with Apple Hardware Test (AHT), you might encounter an alert indicating that the computer does not pass the reading of the following sensor:

4SNS/1/40000001:Ip6M-0.972

Note: "0.972" is a value and may vary.
Resolution

Although the diagnostic displays the alert that the hardware does not pass for this specific sensor, the computer can be considered as passing. The diagnostic tool does not interpret the sensor value correctly. If other messages are also reported, contact AppleCare or your nearest Apple Service Provider.

I was able to reproduce the kernel panic by processing a big image in Photoshop or Illustrator and I suspected this might be a bad RAM so:

1. I produced a kernel panic and made note of the channel 0 and dimm 0 in the panic log.
2. My Mac has 4 DIMM slots.
3. I switched the cards in the first two slots and switched the cards in the last two slots.
4. I produced another kernel panic and noted that the channel had switched to 1 and dimm was still 0.
5. So one of the cards is bad, but which one. Apple's manual only lists them as slots 1, 2, 3, 4. The manual also says 1 & 2 are on there own channel, while 3 & 4 share a channel.
6. So remembering that programming counting often starts at zero instead of one I made the chart pictured.
7. Removed Card 1 - no problems since.

What I can't figure out is why Apple has not included a simple chart like this in the manual. I've seen a lot of posts about people trying to figure out what slot the channel and dimm log statements correspond to, wasting a lot of man hours. Any idea why they can't include such a easy chart - are they afraid to admit that kernel panics occur in the manual or what? I also found nothing on their site.
