How do you create a new instant Messanger category that address book knows is a hotmail address.

I'm not quite understanding what you mean? You have a choice of AIM, jabber, MSN, ICQ and Yahoo.
Is a hotmail address used in an Instant Messenger?

Well, you enter that username into the IM category set as an MSN username. Then you enter it as an email address.
