Perhaps a Permissions Problem

The problem started back in May. My PowerBook (1.67Ghz) running 10.3.9 was hanging up on start. As usual, I got some good comments from the forum but we did not really solve the issue.
Now the problem is very frequent. Frequently when I start or restart the system hangs, usually at the grey screen spinning wheel. (This is guarenteed if I have repared permissions with Cocktail, see below)
There are two solutions to get started:
1. Start in single user mode, mount the disk from ther, then use reboot command. (This only works sometimes)
2. Boot from 10.3 install disk, repair permissions with disk utility, reboot. (This always works ... so far)
Also I find that running the most current Cocktail for 10.3 in repair permissions does not fix the problem, in fact it guarentees a hang. Only repairing permissions with Disk Utility does the trick.
Even when startup is OK there is a consistent permissions problem. Here are the Disk Utility repair permissions results from three consecutive successful restarts:

Perhaps it's a fuzzed directory.
Get yourself DiskWarrior from Alsoft before you get to a point where you won't be able to launch that drive.
DiskWarrior is worth every penny.
It saves me all the time especially after a horrendous crash. Just this morning I had one of drives "disappear". Apple'sDiskUtility "saw" the drive but could not do anything. DiskWarrior "saw" the drive and repaired/replaced the directory and brought it back to the desktop.
Incidently in the problem I had TechToolPro 4.04 was useless. I't could not even "see" the drive.
Cocktail is just doing what Apple'sDiskUtility's repair permissions.
Well that's my suggestion to you jim.

Macworldguy .... I hate to shell out $80 if it's not a directory problem.
Does anyone else see this as a directory problem?
And related, if it's not a permissions problem then why does repairing permissions with Disk Utility always provide at least a temporary fix?
A point to ponder ... if Cocktail just executes Apple's repair permissions then why does repairing permissions with Cocktail result in a hang on the next boot and repairing with Disk Utility does not?

True!
Reading some of the other posts here have you set up CockTail to flush out everything.
Sorry I'm not at my mac computer now and just flying by what this old brain can remember:
I use the "Pilot" in Cocktail and tick everything that's available to me and set CockTail's auto"PILOT" to do a restart and hit RUN. On my 8600 it takes a loooong time and on my QuickSilver it goes in less then 5 minuets.
I would try that since you already have Cocktail.

Just a thought:
After you do the Apple'sDiskUtility repair do you do a restart?
I read somewhere in cyberspace that it's the thing to do.
When I do the "repair" from within my boot volume I run it twice in a row and then do a restart.
I've yet to figure out "how come" that second one comes up with more repairs. It's as if all was not fixed or something is really wonkey. When that happens I give up and boot into DiskWarrior and sure enough there were problems beyond what Apple could fix.

Yes I used to use Cocktail much as you describe. I have now deselected repair permissions under Pilot because every time I use Cocktail (as opposed to Disk Utility) to repair permissions the machine will hang on the next start.
No I've not repaired permissions twice in a row. I'll try that to see if there are problems the second time.
Sometimes I've rebooted right after repair and sometimes not, no differences subsequently noticed.
